• Emergency procedures and crisis resources
• Telehealth and audio-only session guidelines

Your form is securely delivered through Hushmail, a platform fully certified for HIPAA compliance. No personal health information you submit will ever be stored on our website or any non-compliant third-party system.

Returning clients who have already completed their Informed Consent Form do not need to complete it again.
